As you prepare for this exciting new chapter in your academic journey, we want to make sure you have all the information you need to start your university experience with confidence.

Step 1: Understand your medical insurance plan

Result 1: Inpatient Hospitalization
(a) Room & Board - maximum per day
- in case of a non-intensive care room (no max number of days) = 5,000.00฿ (excluded from b)
- in case of an Intensive Care Unit (max 15 days) = 10,000.00฿ (excluded from b)

(b) Hospital General Expenses and Hospitalization Benefit
- maximum per disability = 80,000.00฿

(c) Emergency Accidental Outpatient Treatment
- maximum per disability = 12,000.00฿ (included in b)

(d) Specialist's Consultation Fees
- in case of surgery = 9,500.00฿ (included in Result 2)
- in case of non-surgery = 9,500.00฿ (included in b)

(e) Emergency Ambulance Service Fees
- maximum per disability = 1,000.00฿ (included in b)

(f) Special Nursing Care Fees (no max number of days)
* Fees for Nursing Services are included in Room and Board

Result 2: Surgical Treatment
- maximum per disability (per schedule) = 80,000.00฿ (excluded from b)

Result 3: Physician's Hospital Visit
- maximum per day (max - days) = 2,000.00฿ (excluded from b)

Result 4: Outpatient Treatment
- maximum per 1 visit per day, 30 visits per year = 2,000.00฿

Result 5: Major Medical Treatment (80% coverage)
- Maximum amount of benefit per disability (include in Result 1, 2 and 3) = 600,000.00฿

Result 6: Dental Treatment (100% coverage)
- Covers the actual eligible expenses for non-network dental hospital and clinics
- Maximum per year = 5,000.00฿

Result 7: Personal Accident (50% coverage in case of motorcycle accident)
- Maximum 600,000.00฿ (or Bor.2)

Step 2: Making a claim for medical expenses

Claim documents should be submitted within 30 days of treatment or discharge from a hospital, medical center, or clinic. Original receipts must be submitted (not photocopies).

You will need to submit the following documents:

• A completed claim form
• A medical report stating your symptoms, diagnosis, and treatment. For treatment of skin diseases, the name of the prescription is also required.
• Original receipts containing breakdown of costs*
• A copy of your ID card
• A copy of your membership card

*Important: The company will not process your claim if an original receipt is missing.

Step 3: Sending your claim to the Claim Department

You can submit documents to support your claim by post to :

Step 4: Getting reimbursed

Processing time: If your claim is eligible, we will pay your claim within 15 days of us receiving all documents required to process your claim.

However, in the event that the insured wishes for the company to reconsider the claim result, the company will complete the review within 30 days from the request date for review or receive additional evidence (if any).

What can My Allianz application do?

1. View general policy information checking service, e.g., policy number, coverage, coverage duration, policy status, policy manual or group benefits manual, and insurance plan information.

2. Submit an online claim filing service.

3. Claim information service, e.g., summary of compensation claim rights, history of compensation claims, online claims, annual benefit amount, total amount spent, compensation claim filing information, and compensation claim status, with information in this part going as far back as 1 calendar year from the current year.

4. Access to other services and usage installation services, e.g., password changes, PIN code changes, privacy policy, and expression of consent for disclosing tax information to the Revenue Department.

5. Use a searching service for hospitals located near you.
